If you are a naturalized Canadian citizen or permanent resident and need to replace your lost, stolen, or damaged record of landing document, then this kit will guide you through the process. By using this kit you will be able to have :

The Record of Landing or Confirmation of Permanent Residence is a historical document containing your personal information (name, date of birth, country of birth, etc.) that was relevant at the time you became a permanent resident of Canada. This information cannot be changed at a later date.
To be eligible for a certified copy of your Record of Landing IMM 1000 or Confirmation of Permanent Residence IMM 5292, you must be a permanent resident of Canada or be a naturalized Canadian citizen who once was a permanent resident.

1. What if my documents are not in English?
If the documents you are sending with your application are not in English or French, you must provide a translation and an official declaration from the person who translated the document. Translations provided by family members are not sufficient.

The Application for a Certified True Copy, Correction or Replacement of an Immigration Document (Form IMM 5541) should be used by applications who require a certified or replacement copy of the documents issued by Canadian Immigration authorities because their document contains errors or has been lost, stolen or destroyed.

3. What Types of Documents Can I Request to Obtain?
Certified true copies or replacement copies of the following documents may be requested by you, through the Application for a Certified True Copy, Correction or Replacement of an Immigration Document (Form IMM 5541) :

4. What if I am Applying for More than One Person?
If you are applying for multiple people, such as family members, please use and mail separate applications for each person.

5. Who Do I Contact About My Application?
Depending on your entry date into Canada, different addresses are used to process the Application for a Certified True Copy, Correction or Replacement of an Immigration Document (Form IMM 5541). If you want to obtain a true copy of a document specific to Immigration, and you entered Canada before 1936.
However, if your date of entry was after 1968, please direct your application to Citizenship and Immigration Canada.
